Ways We Can

Partner With You

Ongoing Wellness Support & Programming

Keep people moving forward between appointments, so your staff can spend less time chasing follow-up and more time providing care.

Community Health Events And Engagement

We plan & facilitate your event and engagement top to bottom, so your staff can focus on their responsibilities instead of managing every moving part.

Employee & ERG Wellness

Give employees support they can use during the workday, so HR and managers spend less time putting out fires and more time supporting their teams.

Pick the kind of support your team needs most.

Every partnership is built around your goals, the people you serve, and the results you want to achieve.

Why Organizations Choose HPC

Community Health Events & Engagement

We help shape the event before it starts, plan how people will take part, lead the wellness experience on-site, and review what worked after. Your team gets a smoother event, stronger participation, and clear ideas to make the next event even better.

Ongoing Wellness Programs for The People You Serve

Healthy habits are built after people leave the appointment, not during it. We provide ongoing support that helps people apply what they learned in everyday life while keeping your team focused on the work only they can do.

Employee & ERG Wellness

Whether you need one workshop, a wellness series, a private cohort, or real-time wellness support during work hours, we build and lead the program so employees have practical support when they need it most.

MedCura Health

Their Annual Health Fair Needed More Hands and a Clear Plan.

As MedCura prepared for its annual Back-to-School Health Fair and Family Fun Day, the Community Relations team needed a partner who could help plan the wellness experience—not just show up on event day.

HPC joined the pre-planning team to help map the participant experience, strategize activity placement, build the wellness schedule, coordinate instructors, support event promotion, and lead the wellness engagement from implementation through post-event feedback.

What MedCura Got Back:

  • A trusted partner from planning through implementation

  • Less for staff to coordinate on event day

  • More opportunities for families to participate

  • A smoother event experience for guests and partners

  • Feedback and recommendations to strengthen future events

  • More than 400 community members reached

What Parents Said

“I really enjoyed the event with HPC & MedCura Health! I was able to get a free school physical for my daughter and a backpack full of school supplies. After her appointment I took her to have fun at the bouncy house, slime workshop, petting zoo and painting class. They also provided water and snacks! She is going to sleep really good tonight! Thank you to everyone involved. This was an amazing event for my family and for our community!”

“This back-to-school event was awesome. So many activities and treats for the kids from slime making, bounce houses and a petting zoo. Definitely will come back next year!”

Lashunda’s Patient Support

Featured in Project eCHO as a Holistic Heart Failure Management Initiatives Success Case in GA

Details shared with Permission & words directly from Lashunda.

A smiling woman with glasses and hoop earrings against a pink background.

Her story was featured through Project ECHO Southeast alongside Piedmont Women’s Heart Health Group as a featured Case of community-based holistic programming helping patients carry out their care plan between medical visits.

Her Doctor Kept Telling Her to Exercise. Chronic Pain Made Her Afraid to Start.

Lashunda was living with heart failure, fibromyalgia, chronic pain, and limited mobility. She heard the same advice at every appointment: move more. But working out alone did not feel safe.

Through HPC’s ongoing program, her certified fitness instructor created movement videos just for her body and mobility level. Her health coach helped her work through fear and build a routine she could keep. Through HPC’s optional spiritual support, a licensed minister helped her rediscover hope and purpose during one of the hardest seasons of her life.

What Changed

  • She stopped avoiding movement and started getting excited to workout.

  • She began consistently using the workouts made for her needs.

  • She built confidence exercising safely and has tried new things.

  • She said her body felt better, because “Movement truly was needed… without hurting myself.”

  • She said, “My community has my back.” when discussing the support that helped motivate her.

  • She described HPC as “a sanctuary for healing, growth, and purpose.”

Why Our Whole-Person ModelWorks

Healthy living is harder to sustain when only one part of health is supported.

HPC combines the right parts based on the people you serve, so your program supports more than one piece of their health.

Health Care Partnerships

Closing the Gap Between Clinical Care and Real-Life Outcomes for Women

Before They Fall Through It

Care Shouldn’t Stop at Discharge—But For Most Women, It Does.

We solve low engagement and care gaps in women’s heath, especially caregiver & maternal health programs by providing fully staffed, high-touch engagement systems that keep participants connected, supported, and progressing between clinical visits via our hybrid program model.

Every Partnership Helps More People Stay Well

Her Promise Circle Foundation exists to make whole-person wellness easier to reach before stress, illness, or burnout becomes a crisis.

When you partner with HPC, you help more people get the support they need while giving your staff more time to focus on the work only they can do.

Let’s Build the Right Partnership.

Tell us who you serve, what you are trying to improve, and where your team needs help.

You do not need to have the full plan figured out before we meet.
