Post-Discharge & Preventive Wellness Support
Maternal Wellness, Recovery & Prevention
Program Overview
Our Post-Discharge & Preventative Wellness Support focuses on helping women successfully transition from clinical care into daily life—while also reducing the risk of chronic disease progression and future complications.
This community-based approach supports women after discharge and those at risk by providing structure, education, and real-life tools to follow through on care plans, build sustainable habits, and maintain long-term health stability.
By addressing both recovery and prevention, we help healthcare organizations close the gap between clinical care and real-life outcomes—where consistency and support are critical.
-
In DeKalb County and the greater Atlanta area, many of the leading causes of hospitalization are directly influenced by lifestyle, stress, and lack of ongoing support after discharge:
Major cardiovascular conditions account for 1,124.6 hospitalizations per 100,000 residents, making them the leading cause of hospitalization
Stroke-related hospitalizations reach 240.3 per 100,000 residents
Chronic diseases are defined as long-term conditions that often require ongoing management and are frequently preventable through lifestyle, stress management, and early intervention
While clinical care addresses acute needs, many women leave the hospital without the support needed to regulate stress, rebuild strength, and integrate care plans into daily life.
This program addresses that gap by focusing on real-life implementation—supporting women through stress regulation, movement, recovery, and sustainable habit-building so they can follow through and maintain stability beyond discharge.

Maternal Wellness
Maternal Wellness, Recovery & Prevention
Program Overview
Our Maternal Wellness & Recovery Program is a community-based initiative designed to support underserved mothers’ mental, physical, and emotional health before, during, and after pregnancy.
Through education, movement, and stress regulation, we help women successfully navigate recovery and maintain stability during one of the most vulnerable periods of care.
This program addresses critical gaps in maternal health by providing accessible, preventative wellness support—extending care beyond clinical settings and into real life.

Maternal and infant health remain priority concerns in DeKalb County and across Georgia.
Key maternal health challenges include:

Local public health assessments in DeKalb County identified mental health and maternal/infant health as priority community concerns, with 89% of residents reporting the need for expanded mental health services and support for new parents.
Community recommendations emphasized the need for:
