Post-Discharge & Preventive Wellness Support

Maternal Wellness, Recovery & Prevention

Program Overview

Our Post-Discharge & Preventative Wellness Support focuses on helping women successfully transition from clinical care into daily life—while also reducing the risk of chronic disease progression and future complications.

This community-based approach supports women after discharge and those at risk by providing structure, education, and real-life tools to follow through on care plans, build sustainable habits, and maintain long-term health stability.

By addressing both recovery and prevention, we help healthcare organizations close the gap between clinical care and real-life outcomes—where consistency and support are critical.

  • In DeKalb County and the greater Atlanta area, many of the leading causes of hospitalization are directly influenced by lifestyle, stress, and lack of ongoing support after discharge:

    • Major cardiovascular conditions account for 1,124.6 hospitalizations per 100,000 residents, making them the leading cause of hospitalization

    • Stroke-related hospitalizations reach 240.3 per 100,000 residents

    • Chronic diseases are defined as long-term conditions that often require ongoing management and are frequently preventable through lifestyle, stress management, and early intervention

While clinical care addresses acute needs, many women leave the hospital without the support needed to regulate stress, rebuild strength, and integrate care plans into daily life.

This program addresses that gap by focusing on real-life implementation—supporting women through stress regulation, movement, recovery, and sustainable habit-building so they can follow through and maintain stability beyond discharge.

      • Improve patient follow-through after discharge

      • Reduce preventable complications and hospital readmissions

      • Support chronic disease management and early intervention

      • Strengthen alignment between clinical care plans and daily life

      • Increase access to preventative wellness support in underserved communities

      • Empower women with tools to sustain long-term healthIncrease maternal health knowledge and self-care skills

    • Strengthen peer support networks and reduce isolation

  • Her Promise Circle Foundation’s preventative wellness programming is already demonstrating measurable outcomes.

    Recent participant survey data shows:

45 women participated in preventative wellness programming in Q1 2026
100% reported reduced feelings of overwhelm
95% reported improved ability to regulate stress
68% reported applying wellness tools to daily challenges

These early outcomes demonstrate the effectiveness of community-based preventative wellness education and support in helping women manage stress and strengthen their overall well-being.

    • Increased adherence to post-discharge care plans

    • Reduction in avoidable complications and readmissions

    • Improved stabilization and management of chronic conditions

    • Increased engagement in preventative care and wellness practices

    • Improved physical, mental, and emotional well-being

Maternal Wellness

Maternal Wellness, Recovery & Prevention

Program Overview

Our Maternal Wellness & Recovery Program is a community-based initiative designed to support underserved mothers’ mental, physical, and emotional health before, during, and after pregnancy.

Through education, movement, and stress regulation, we help women successfully navigate recovery and maintain stability during one of the most vulnerable periods of care.

This program addresses critical gaps in maternal health by providing accessible, preventative wellness support—extending care beyond clinical settings and into real life.

  • Maternal and infant health remain priority concerns in DeKalb County and across Georgia.

    Key maternal health challenges include:

1 in 5 mothers experience prenatal or postpartum depression or anxiety

    • Many women receive minimal recovery guidance after medical clearance at six weeks postpartum

    • Chronic stress during pregnancy is associated with preterm birth, low birth weight, and delivery complications

    • Social isolation and lack of support increase maternal burnout, mental health challenges, and reduced self-care

Local public health assessments in DeKalb County identified mental health and maternal/infant health as priority community concerns, with 89% of residents reporting the need for expanded mental health services and support for new parents.

Community recommendations emphasized the need for:

    • Increased mental health services for new and expecting parents
    • Affordable or free prenatal and postnatal support programs
    • Community-based wellness and education resources

These findings demonstrate a clear need for accessible preventative maternal wellness programs that support mothers beyond clinical care settings.

    • Improve maternal mental health and emotional resilience

    • Support safe postpartum physical recovery and injury prevention

    • Reduce stress-related pregnancy and postpartum complications

    • Increase maternal health knowledge and self-care skills

    • Strengthen peer support networks and reduce isolation

  • Her Promise Circle Foundation’s preventative wellness programming is already demonstrating measurable outcomes.
